My flashlight icon disappeared

My flashlight icon disappears and appears by it self from the active quick settings bar although I did added it and never removed the list

restart the phone > pull down the notification bar > tap on the edit icon > see if you find it

When you tap on the Edit icon > you do get a bar to drag down icons correct? what if you swipe left <> right until you find it?

I can't see the screenshot just yet, btw you can take screenshots by pressing power & volume down at the same time. 

Do you experience the same on >>> safe mode?

You could repair the phone with PCC, Bridge or XC to narrow down the problem.
