This could be an issue with Lollipop OS, but since I have Xperia device, I decided to come here.
I have Unlocked Xperia Z3 Compact(with latest OS .128) and in settings>security>smart lock>Trusted places I have set my Home as trusted place. Works fine and I don't have to constantly unlock my device when at home.
Set the lock screen passcode(mine is a pattern)
1) Trusted Place is set as Home
2) Stamina mode is on
3) Make sure that phone works fine in trusted place and user doesn't have to enter pattern/pin every time.
4) Without waking up the phone, go to another location(can drive couple of km away from trusted place).
5) Once in another area, press power button or double tap the screen, then swipe to unlock to see if phone asks for any lock.
Here is the issue: When I leave the trusted area and lets say go to work, upon pressing power button I am able to just swipe on the lock screen and don't have to enter password. It happens only once, because after that first wake up, phone realizes that it is not in trusted area anymore. I think that with stamina mode and when phone is in standby it isn't checking to see if user left trusted area, which leaves device unprotected. I see this issue 80% of the time.
Anyone one with this issue?

Safe mode - do you mean stamina mode? I have not tried yet.
I have done some testing:
1) When user has Stamina mode on - phone may not ask for lock pin/pass even if user is out of trusted place.
2) When Stamina mode is off - phone asks for pin/pass when user leaves trusted place.
Speculating here.. but here it's my story.
When I am at home, it works as designed, when I am at works same. But when I am at a friend house who live in an apartment at 2nd of 8 floors there it won't works. Reason? Seems that there is no GPS signal and that the network supported location service fail to return a decent location.
.. may it be the same issue you are having? My phone is set in "High accuracy" and also notice the first message appears when you want to set up a trusted place: Keep in mind that location accuracy can't be guaranteed. Trusted places has a range of 80 meters.
